    Job Description

    The Registered Nurse plans, organizes and directs homecare services and is experiences in nursing, with empathy on community health education/ experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

    Responsibilities:

    • Administer nursing care to ill, injured, or disabled patients
    • Diagnose and establish patient treatment plans
    • Monitor and report changes in patient symptoms or behavior
    • Communicate with collaborating physicians or specialists regarding patient care
    • Educate patients about health maintenance and disease prevention
    • Facilitate referrals to other healthcare professionals and medical facilities
    • Maintain accurate patient medical records
    • Provide advice and emotional support to patients and their family members
    • Regularly evaluates patient nursing needs
    • Initiates the plan of care and necessary revisions as patient status needs
    • Provides health care instructions to the patient as appropriate per assessment and plan of care
    • Identifies discharge planning needs as part of the care plan development and implements prior discharge of the patient

    Qualifications:

    • Registered nurse with current licensure to practice professional nursing in the state
    • Graduate of National League for Nursing accredited school of nursing
    • Maintains current CPR certification
    • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
    • Minimum two years of experience, at least one which is in the area of public health or home care nursing is preferred
    • Self-directed and able to work with minimum supervision
    • Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist
    • Shows ability to organize and prioritize workload independently
    • Responsible for supervising Home Health Aides
    • Prolonged or considerable standing or walking
    • Able to lift, position, or transfer patients. Able to life supplies and equipment
    • Previous experience in nursing or other medical fields
    • Familiarity with medical software and equipment
    • Ability to build rapport with patients
    • Strong problem solving and critical thinking skills
    • Ability to thrive in a fast-paced environment
