Continuum Home Healthcare is seeking an experienced
home healthcare aide to join our team to service clients in the
Holland, Michigan area.

In this position, you will be responsible for providing personalized in-home care and support to elderly and disabled clients.

Clients are located in the
Holland, Michigan area and
hours would range between 10am-8pm or second shift hours. As a
home healthcare aide, you will play a crucial role in ensuring the well-being and comfort of our clients.

Responsibilities:


  • Assist clients with activities of daily living such as bathing, dressing, grooming, running errands, and meal preparation
  • Monitor clients' health and wellbeing, and report any changes or concerns to the care management team
  • Engage clients in conversation and activities to provide companionship and improve their quality of life
  • Maintain detailed records and documentation related to client care in AxisCare

Requirements:

Minimum 2 years experience working in home care, nursing, or caregiver role
preferred

  • Excellent communication and interpersonal skills
  • Compassionate, patient, and empathetic personality

Reliable transportation and valid driver license is required to travel to client homes within the Holland, MI area.

  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA)
    certification is not required
