Job Description
Job DescriptionThe LPN provides direct and indirect nursing care in a family practice setting, or in a school-based health setting. The LPN will participate in all aspects of care to patients including triage, health maintenance teaching, and administration of vaccines/medication. The LPN will also collaborate and communicate with other nursing professionals, medical assistants, and medical providers to ensure high quality patient care. The LPN will provide support to medical providers, as needed in the management of prescriptions and controlled substance prescriptions.
Educational Requirements:
Graduate of Accredited LPN program required.Experience Required:
Preferred candidate will have two years of experience in nursing role in an ambulatory care setting. Knowledge and proven skill in chronic care management or pain management is preferred. Professional Licensure/Special Skills and Certification Required:Current licensure as LPN by Massachusetts State Board of Registration BLS required.

1. Commitment to providing health care to families in a medically underserved community.
2. Willingness to be flexible in the development of new methods of health service delivery.
3. Ability to deal with sensitive information in a highly confidential manner.
4. Effective communication skills, both oral and written, with patients, families, visitors, co-workers, health care professionals, funders and other regulatory agencies.
5. Bilingual in English and another language appropriate to the health center's patient population preferred, but not required.
6. Ability to function independently with minimal supervision, and to plan, coordinate, and carry out those tasks necessary to meet the job responsibilities.
7. Awareness of resources available within the health center to accommodate the needs of patients.
8. Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care.
9. Processes personal health information in accordance with FHCW HIPPA policies and procedures.
Supervisory Responsibilities:
May be asked to provide guidance to medical assistants or to administrative staff by providing direction with procedures and other activities to facilitate patient care.Reporting Relationship:
Reports directly to the area Team Lead Nurse
