    REPORTS TO: Housing Manager

    HOURS & LOCATION: 1 (FTE) 40 hours per week (non-exempt), Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m., may require occasional late hours

    SALARY: $26.50

    SCOPE:

    The peer support specialist will be responsible for the development and on-going provision of peer support services within the participants of Project HAVEN, a scattered site permanent supportive housing program for formerly homeless African Americans. The Peer Support Specialist provides a variety of recovery-oriented health, stability and wellness-focused services to individuals who have self-identified as desiring peer supports. The peer support specialist seeks to promote greater independence, community integration andrecovery from mental illness and substance abuse disorders to participants in our permanent supportive housing. Services provided by peers (mental health participants with lived experience) include a wide range of supports, services, and advocacy that contribute to participant's ability to engage in ongoing treatment.

    Requirements

    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    • Provide supportive services in a culturally-responsive, empowering, and non-judgmental manner.
    • Facilitate the development of recovery skills among assigned participants as a member of the Project Haven support team. Serve as a mentor/role model demonstrating competency in recovery, effective coping skills and self-help strategies for African Americans.
    • Serve as a participant advocate, provide participant information and provide support for participants during emergencies as well as in outpatient, inpatient and mental health diversion settings.
    • Function as Peer Support Specialist for assigned cases.
    • Assist participants in articulating personal goals for recovery.
    • Assist participants in determining their treatment objectives.
    • Teach and support acquisition and utilization of skills needed to facilitate individual recovery.
    • Promote the knowledge of available service options and choices.
    • Observe the progress participants make toward meeting objectives.
    • Develop and conduct recovery action planning in collaboration with housing specialist and other relevant service provider (internal or external). Educate and engage individuals in the recovery process as a means to recognize early triggers, signs of relapse, and use of individual coping strategies.
    • Assist with teaching independent living skills such as accessing public transportation options, cooking, cleaning, accessing and maintaining stable housing, personal finances, etc. Recommend and make referrals to programs for instruction on life skills, livelihood skills and workplace readiness skills necessary for successful reintegration into family life, the work force and the community. May instruct participants in group settings, individually or in their homes as appropriate.
    • Maintain caseload (up to 30 individuals). Monitor and keep case records on clients and report progress to supervisors; participate in well checks, retention calls, and other check-ins as assigned.
    • Maintain all records in accordance with the standards established Urban League standards, as well as all local, state, and federal laws and accreditation guidelines. Assure that all work is properly documented in participant records, or elsewhere, as required.
    • Perform quality assurance activities as assigned.
    • Recognize that individuals with co-occurring disorders are expected, not the exception, and are welcome into treatment.
    • Assist in transporting and attending with participants internal and community based meetings as needed.
    • Provide participants with information about community resources and keep current on referral resources.
    • Ensure all services provided comply with professional standards, state and federal regulations and thepolicies and procedures. Understand and follow all applicable policies and procedures including those regarding confidentiality and HIPAA regulations.
    • Perform other related duties as assigned.
    EDUCATION AND WORK EXPERIENCE AND OTHER REQUIREMENTS
    • High School Diploma or GED , Required
    • Bachelor's degree in Behavioral Sciences field or a combination of at least three year's work, education, training and/or experience in chemical dependency or mental health treatment, Preferred
    • Community Health Worker Certification (or ability to complete certification process), Required.
    • Peer Support Specialist Certification (or ability to complete certification process) Required.
    PLUS:

    Must be a current or past participant of mental health or co-occurring behavioral health services. Applicant must identify on the cover letter how they have "lived" personal experience leading to an understanding of the needs of African Americans with mental health & substance abuse disorders.
    • Minimum of two years success within personal recovery model.
    • Ability to utilize individual and group skills.
    • Strong knowledge of local community behavioral health services.
    • Must possess and maintain a valid driver's license and own mode of transportation at the time of hire and throughout the course of employment.
    KNOWLEDGE, SKILLS, AND ABILITY REQUIREMENTS:
    • Knowledge of local community behavioral health services, community agencies and support groups.
    • Knowledge of public transit system preferred.
    • Knowledge and skills sufficient to use community resources necessary for independent living and to teach those skills to those participants.
    • Proficiency in the operation of a personal computer including standard business applications Microsoft office suite including touch screen mobile devices and standard office equipment (copy, fax, scan).
    • Ability to publically acknowledge and share personal recovery experiences and provide support to otherpersons-in-recovery.
    • Ability to facilitate individual and group meetings.
    • Ability to successfully integrate into the current treatment team process.
    • Ability to work independently.
    • Ability to communicate effectively in both oral and written manner.
    • Ability to establish and maintain professional relationships with participants, co-workers, supervisors, participant family members and the general public.
    • Ability to handle and maintain confidential information in conformance with HIPAA guidelines.
    • Ability to work varied days and hours including evenings and weekends as needed.
