Overview:
We are seeking a dedicated and compassionate Case Coordinator to join our team.

As a Case Coordinator, you will play a crucial role in providing support and coordination for patients in need of behavioral health services.

Duties:


  • Conduct diagnostic evaluations to assess patients' needs and develop appropriate treatment plans
  • Coordinate and facilitate discharge planning for patients transitioning from acute care settings
  • Collaborate with healthcare professionals, including social workers and medical staff, to ensure comprehensive patient care
  • Provide case management services to patients, including coordinating appointments, referrals, and followup care
  • Assist with patient service coordination, ensuring timely access to necessary resources and support
  • Maintain accurate and uptodate documentation of patient interactions and progress
  • Utilize medical terminology and knowledge of behavioral health practices to effectively communicate with patients and healthcare providers

Skills:


  • Bachelor's degree in Social Work or related field preferred
  • Experience in behavioral health or related field is highly desirable
  • Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously
  • Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare professionals
  • Knowledge of diagnostic evaluation processes and discharge planning procedures
  • Familiarity with case management principles and practices
  • Ability to provide compassionate patient care while maintaining professionalism
  • Proficiency in medical terminology related to acute care settings

Pay:
$ $22.00 per hour
