***This position serves as an outpatient clinical liaison for general cardiology & electrophysiology, pre- and post-cardiac procedures, interventions and surgical patients. Roughly 90% back-office and 10% patient facing duties are incorporated including but not limited to coordination of procedures, and follow-up appointments, pre
- and post-operative education, triaging clinical inquiries and wound care and education.
POSITION RESPONSIBILITIES
- Determines appropriate and indicated level of care and delegates
- Collaborates and communicates with physicians, advanced practice providers, medical assistants, and backoffice team to ensure quality care
- Helps to develop, coordinate, and educate on the patient's continuity of care based on the patient/ caregivers learning style
- Sees patients in clinic as needed to assess wounds and provide routine and complex wound care, suture/ staple removal and education for postsurgical care to patients who have undergone a vascular surgeries and procedures
- Review, communicate, and educate on laboratory results, imaging studies, medication refills, and medication changes with patients per protocol and at physician's request
- Complete vascular surgery clearance letters, FMLA/ disability forms, cardiac rehab, or home health orders or requests
- Review external medical records and update electronic medical records appropriately
- Complete and document medication reconciliation, medical/ surgical/ social history documentation
- Document patient's plan of care and interventions in electronic medical record, according to policy
- Assists with Enhanced external Counterpulsation (EECP) therapy
- Maintains and demonstrates competency in BLS (Basic Life Support), infection control, safety, and all required skill reviews
- Utilizes interventions substantiated by research as appropriate to the practice environment

Education & Licensure
- Required: Licensed Vocational Nurse (LVN) or Registered Nurse (RN) active license
Minimum Experience Required:
None
- Preferred: Cardiology / Electrophysiology experience
Minimum Knowledge, Skills & Abilities
- Demonstrate the knowledge and skills necessary to provide care appropriate to the patients served
- Demonstrate critical thinking and independent decisionmaking
- Good communication skills with adult & older adult populations
- Familiarity with PC's & computerized nursing systems and tools or demonstrated ability to learn
- Strong interpersonal skills to interact positively and effectively with patients, visitors, staff, and physicians
- Good organization, delegation, and communication skills to facilitate efficient patient care
- Strong analytical decision making and critical thinking skills
- Good leadership and coordination skills
