    Position Description:

    Within the regulatory scope of practice, the LVN Case Manager will support the delivery of direct patient care by conducting routine patient care procedures, technical tasks, and providing supportive functions in the clinical setting for clients enrolled in JWCH mental health programs.

    Principal Responsibilities:
    1. Performs routine patient care functions as an LVN within the defined scope of practice, including medication management, treatment compliance, and skill building for memory and self-sufficiency.
    2. Administers medications by injection as determined by the treating provider. Checks prescribed dosages against specified limits. Observes and reports adverse reactions to medication as deemed necessary.
    3. Uses and practices basic assessment (e.g., data collection), patient care, and documents pertinent patient information and data in accordance with the policies and procedures of JWCH and the standards of LVN practice.
    4. Works within a multidisciplinary team to support clients diagnosed with mental illness and co-occurring conditions through medical coordination, assessment, and accompaniment.
    5. Develops patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
    6. Provides consultation to and coordinates care for patients needing specialty mental health and/or substance use disorder services.
    7. Maintains standards of cleanliness, organization, and an adequate level of supplies for the assigned work area.
    8. Participates in assigned staff and other meetings, including quality improvement, case conferences, and in-service meetings.
    9. Conducts screenings for all service requests in a timely manner (as defined by DMH Access to Care standards).
    10. Completes all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation.
    11. Participates in structured and as-needed case conferences.
    12. Assists with provider education for integrated behavioral and primary care services.
    13. Provides ongoing case management services, including linkage and referral to both internal programs and external resources.
    14. Supports clients in building skills to reduce impairment from medical and mental health illnesses.
    15. Provides field-based care as deemed necessary for client treatment.
    16. Performs other duties as assigned.
    Requirements:
    1. Current and valid California LVN License
    2. Current BLS card from the American Heart Association.
    3. At least one-year experience in mental health setting or working with mentally ill population.
    4. Ability to communicate effectively orally and in writing.,
    5. Experience in working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds.
    6. Current and valid California Driver's License with current car insurance and a clean driving record.
    7. Bilingual (English/Spanish) is preferred.
