    Job Description

    Compass Healthcare Consulting and Placement is currently looking for an experienced Registered Nurse for a Hybrid RN Care Management Team Lead position at Managed Care Plans. This role is a mix of remote work (2 days) and in-office work (3 days) in Manhattan, NY. Candidates need to have a NYS-RN License, UAS-NY Certification, previous supervisory experience in RN Care Management, Documentation Review, and MLTC. It's important to be able to commute to Manhattan, NY 3 days per week.

    The Care Management Team Leader oversees the Care Management Teams' clinical supervision and overall team performance to fulfill duties and meet contract requirements, managing 10-12 teams.

    Qualifications:

    • RN (BSN Preferred) with a current, valid, and unencumbered license
    • 5 years of clinical experience with at least 1 year in home health
    • Proficiency in MS Office, strong verbal and written communication skills, ability to work well under pressure
    • Compliance with state and federal licensure and employment requirements
    • Additional Requirements: Bi-lingual, UAS certified, knowledge of care management

    Essential Job Responsibilities:

    • Proficiency in article 49 regulation
    • Manage service authorizations and care plans for high-risk populations
    • Coordinate Hospitalization / Skilled Services CHHA / Nursing Home communication and planning
    • Benefits coordination for Medicare, Medicaid, and other insurance
    • Transitional care management coordination
    • Complete Monthly Call Assessments and follow-up on necessary calls
    • Coordinate post-discharge services
    • Implement long-term care quality measures and clinical pathways based on evidence
    • Collaborate with the Clinical Consultant Team for complex member needs
    • Document all interventions and findings

    Salary is between $120,000 to $125,000 per year, depending on experience.
