    Description

    Harvest Pointe is looking for a Social Service Coordinator!
    Are you passionate about assisting elderly residents with accessing essential services and resources? If yes, this might be the perfect opportunity for you!


    JOB SUMMARY:
    - Identify residents in need of services and coordinate service delivery.
    - Establish linkages with community service providers.
    - Develop and maintain health & wellness programs.
    - Compile a list of community resources for residents.
    - Serve as a resident advocate and teach self-advocacy.
    - Liaison with community agencies and seek new services for residents.
    - Maintain positive relationships between residents and service providers.
    - Advocate for residents with management and other departments.


    KNOWLEDGE & SKILLS:
    - Familiarity with supportive services and community resources.
    - Strong advocacy and problem-solving abilities.
    - Uphold ethical standards and client confidentiality.
    - Excellent communication skills.
    - Ability to work independently with excellent time management.
    - Proficiency in Microsoft 365 tools (Word, Excel, PowerPoint & Outlook).


    EDUCATION AND EXPERIENCE:
    Bachelor's degree in Social Work, Gerontology, Human Services, Psychology, or Sociology preferred.
    Candidates with relevant work experience are encouraged to apply.
    Experience with seniors and disabled individuals is a bonus.
