- Plan, coordinate and manage treatment plans and interventions for patients of all age groups in accordance with the Patient Centered Medical Home/ Specialty Practice expectations.
- Conduct assessments and determine patient/family needs, develop interventions and create an effective discharge plan.
- Coordinate with the care team, physician and other staff to ensure quality patient outcomes are achieved in an efficient manner.
- Collaborate with interdisciplinary teams to ensure continuity of care and facilitate health service delivery.
- In this full-time role, travel is not required and shifts are on a daytime schedule.
- Qualifications: Graduate of a nationally accredited nursing program, 1 year of directly related experience. New Mexico RN license and CPR/BLS certification required.
