- Review patient cases to determine necessary medical interventions
- Collaborate with healthcare team members to create and implement treatment plans
- Conduct assessments of patients' clinical and psychosocial status
- Coordinate with physicians, nurses, and external resources to ensure quality care and cost-efficient utilization of resources
- Maintain accurate and timely documentation of patient care activities
- Participate in interdisciplinary team meetings
- Ensure compliance with hospital policies and procedures
- Graduate of a nationally accredited nursing program
- Current license as a Registered Nurse within the state of hire
- 1 year of nursing experience
- CPR certification
- Ability to effectively communicate with patients, families, and healthcare team members
- Light work, requiring the ability to lift up to 20 pounds occasionally and up to 10 pounds frequently
- May require sitting, standing, or walking for extended periods of time
- Requires manual dexterity and visual acuity

Nurse Case Manager - Per Diem
We are seeking a Nurse Case Manager to oversee patient care and ensure quality outcomes. The Nurse Case Manager will manage treatment plans, collaborate with healthcare professionals, and engage patients and their families to ensure their needs are met. Additionally, the Nurse Case Manager will be responsible for coordinating communication with various departments of the facility, community organizations, and external resources in order to provide comprehensive patient-centered care. The ideal candidate will have a minimum of 1 year of nursing experience and be licensed as an RN in the state of hire.
