- Identify patients requiring case management interventions
- Perform patient assessments and collaborate with healthcare team members and others
- Develop comprehensive, patient-centered plans of care
- Coordinate referrals for home health, infusion, and equipment services
- Monitor and evaluate patient responses to interventions
- Ensure continuity of patient care across all settings
- Participate in continuous quality improvement activities
- Maintain accurate records and documentation
- Encourage patient participation in their own care and safety
- Graduate of a nationally accredited nursing program
- Current RN license in New Mexico or under reciprocal agreement
- Current BLS certification
- At least one year of directly related experience
- Light work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Physical demand requirements are in excess of those for sedentary work.
- May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.
- Minor hazard: physical risks, dirt, dust, fumes, and noise.
