Under supervision, to provide responsible, quality clinical treatment to chemically-addicted patients
Specific Duties and Responsibilities:
Counseling patients admitted to programs within the Chemical Dependency Division. Accepted current knowledge-based skills in the treatment of substance use disorders. Identifies specific problems of patients; is sensitive to cultural and spiritual needs of patients. Listens to patient's complaints and promptly reports them to immediate supervisor. Evaluates the progress/or lack of progress of patients. Develops patient treatment plans and establishes short-term and long-term goals in consultation with clinical treatment team. Participates in clinical team meetings in discussing individual cases and general patient progress. Develops accurate psychosocial history of patients. Conducts groups and psychoeducational training sessions with patients. Maintains accurate, confidential, and legible medical records on each patient according to agency requirements. Initiates and completes OASAS ADMISSION/DISCHARGE (PAS 46) forms. Initiates discharge planning and referrals, acting as liaison for patient and completes DischargeSummaries.
Is knowledgeable of Hospital, Departmental and unit Policies and Procedures, and Demonstrates the ability to perform his/her duties in accordance.
Conducts overdose prevention training and distributes naloxone rescue kits.Is courteous and professional when interacting with colleagues, patients and the public Other tasks as determined by department need and the Director of Chemical Dependency Services Is knowledgeable of hospital and departmental policies and procedures, and demonstrates the ability to perform duties in accordance.
Minimum QualificationsAny individual working in an OASAS-Certified Treatment Program or newly-hired into the title of AssistantAddiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS"Supporting Recovery with Medications for Addiction Treatment (MAT)" training and "Introductory Training in theAddictions" training.1.
Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC)High School diploma or its educational equivalent and two (2) years of full-time paid satisfactory experienceperforming case management, care coordination, and related supportive functions in a substance usedisorder treatment program; or4.
Satisfactory equivalent of education, training, and experience. However, all must have at least a High Schooldiploma or its educational equivalent.Note:
