If you are passionate about providing high quality, evidence-based care for individuals in need through an innovative practice, then Forge/KPC is the right fit for youWho we're looking for:We are looking for an exceptional Full Time Addiction Counselor to join our growing clinical team in our facility located in Rego Park, Queens, NY The Addiction Counselor will be responsible for the direct treatment of an assigned caseload of patients who are addicted to alcohol or any other mind/mood altering substances.
Please note, this position will be fully in person and reports into the Executive Clinical Director of the KPC team.
This is a full time role, with a schedule of Monday-Thursday 12-9pm. The compensation range is $26-30/hr.What you'll be doing:
Provide group counseling to an assigned caseloadProvide individual counseling as neededCompletes assessment and intake as scheduledDesigns and implements treatment plansResponsible for documentation of all services providedMaintains contact with referral sources, including EAP's and Criminal Justice referral sourcesGenerates and sends Referral Progress ReportsParticipates in weekly Case Conference meeting for treatment review of patientsAttends weekly supervisionParticipates in staff meetingsAssures that KPC's policies and procedures and federal regulations regarding confidentiality of the patient and their records are strictly adhered toPerform other duties as assigned which include but not limited to managed care reviews, care coordinationWhat you'll need:Education & Qualifications:
