    Responsibilities

    The Physical Therapist role involves delivering physical therapy services, collaborating with other healthcare professionals, and overseeing non-professional home health aide staff in providing patient care. As a Physical Therapist, you will:
    • Provide comprehensive physical therapy services, including evaluation, goal-setting, teaching, and training as per the patient's Care Plan
    • Regularly update care plans to reflect changes and recertification requirements
    • Submit timely visit notes as requested by the Clinical Supervisor
    • Conduct OASIS assessments and discharge planning in accordance with Medicare guidelines
    • Participate in team meetings, conferences, and educational sessions

    Qualifications
    • Active State Physical Therapy License
    • Valid driver’s license, clean driving record, insurance, and reliable transportation
    • Minimum one year experience as a home care Physical Therapist, with proficiency in assessments and OASIS-C
    • Strong clinical judgment and ability to develop individualized care plans
    • Excellent clinical skills and familiarity with Microsoft Office
    • Comfortable with daily driving of 2-4 hours
