    Responsibilities

    What you'll do:

    • Coordinate the overall interdisciplinary plan of care for patients from admission to discharge.
    • Create a care plan integrating patient/family preferences and values.
    • Assess, evaluate, and recommend treatment for various needs.
    • Advocate for resources and removal of barriers.
    • Maintain ongoing dialog for effective health plan implementation.
    • Act as a resource for medical record documentation and post-discharge care services.

    Requirements

    What you'll need:

    • One of the following required:
      • Clinical Counselor certification
      • Bachelor Social Worker certification (Tennessee)
    • Case Manager certification from ACMA preferred.
    • Case Manager certification from CCMC preferred.
    • Education requirements: High School diploma, Associate's degree, or 4 years of specific job experience.
