The RN Care Manager is a member of the care team responsible for all day-to-day coordination and management of care needs and care plans for our local offices in Florence and Coos Bay. We are looking for a proven leader who's committed to building the future of in-home care by delivering a best-in-class experience for clients, caregivers, and our staff.
You will be responsible for thinking through the client journey and how we can continue to optimize the client onboarding experience from inquiry to assessment to client care and connection.
