    Job Description


    Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai'i.

    With over 200 physicians who are leaders in their fields, Straub provides its patients with diagnoses and treatments for more than 32 different medical specialties, including bone and joint, heart, cancer, endocrinology/diabetes, family medicine, gastroenterology, geriatric medicine, internal medicine, vascular and urology.

    Straub is home to the Pacific Region's only multidisciplinary burn treatment center.

    The hospital consistently brings new technologies and innovative medical practices to Hawai'i, such as minimally invasive cardiac surgery and total joint replacement.


    Straub's Kailua clinic brings new services and larger facilities women's health, family planning, ophthalmologic exams, glasses and contact lenses, minor skin surgery and ambulatory care.

    Administrative staff support the facility with warm customer service, competency and fiscal responsibility.


    As a Community Health Worker, you will provide a variety of duties to engage, support, advocate, and educate community members in overall health and wellness.

    In addition, you will serve as a liaison between health and social services in the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activiaties such as outreach, community education, social support, and coordination.

    This unique role works in both clinical and community based settings, including client's homes.

    Minimum Qualifications:
    High school or equivalent. Community Health Worker (CHW) Certification. Current BLS/HCP card.

    Current driver's license and valid Hawai'i auto insurance with short notice access to an automobile to conduct site visits, as required.

    One (1) year of experience in related field.

    Hawai'i Pacific Health offers a comprehensive and competitive total rewards package that includes pay and benefits. The pay rate, if displayed, is the minimum hourly rate for the position.

    Rate of pay for selected candidates will be determined by various factors including knowledge, skills, abilities, relevant experience and training, as well as internal peer equity.

    Starting Hourly Rate: 20.73
