- Conduct comprehensive assessments of clients' physical, emotional, and social needs.
- Develop individualized treatment plans in collaboration with clients and their support network.
- Coordinate and monitor the delivery of services and interventions to ensure continuity of care.
- Provide counseling, support, and education to clients and their families.
- Facilitate group therapy sessions focusing on addiction recovery and relapse prevention.
- Maintain accurate and up-to-date documentation of client progress and treatment plans.
- Collaborate with healthcare professionals, community resources, and external agencies to optimize client care.
- Participate in case conferences, team meetings, and continuing education activities.
- Adhere to ethical and professional standards in all aspects of job performance.
- Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field.
- Certified Addiction Counselor (CAC) or equivalent certification preferred.
- Experience working in addiction treatment or related healthcare settings.
- Strong understanding of addiction recovery principles and evidence-based practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively in a multidisciplinary team environment.
