- Bachelor's Degree in a Social Work, psychology, or a human services related field OR Three years of experience working with at risk youth, vulnerable populations - preferably homeless populations
- Familiarity with social services and support agencies in the District
- Strong communication and organizational skills required
- Reporting to the Program Manager, this is a full-time, exempt level position. All candidates must possess a valid driver's license and have reliable transportation; pass the required clearances; and provide any additional information regarding previous records, if necessary.
- Completes all necessary funder specific admission processes with clients when referred to Thrive including intake, program rules and policies (confidentiality, bullying, release of information, etc.).; ensures SPDAT assessments are up to date;
- Using a competency based framework and in collaboration with the treatment team at Thrive - creates, implements and monitors an individual service plan to assist clients in meeting their goals towards self sufficiency;
- Connects clients with appropriate social services and actively supports them in the navigation of services including vital documentation, food supports, relevant benefits, etc.; connects clients with relevant resources to their individual service plans and actively supports them in cultivating those relationships;
- Conducts individual counseling sessions with clients on caseload;
- Supports the reporting, planning and implementation of activity schedule at the program including various life skills, workshops and trainings on relevant topics to clients;
- Support or provide crisis intervention and on-site counseling when appropriate;
- Perform regular house check inspections inside the building and in the area immediately outside; monitor facilities and enforce program rules for client safety;
- Help provides office coverage, answer and record all phone calls and visitors;
- Complete service documentation in client case records within 24-48 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes
- Participates in regular team consultation meetings with the Behavioral Health Clinician, housing specialist, employment specialist and other staff working with shared clients
- Participate in staff meetings, trainings, conferences, regular supervision and community outreach events as required; taking special care to ensure that 40 hours of required training are completed annually
- Availability in evenings and weekends as needed;
- Other duties as assigned by the Program Manager, the Chief of Residential Programs or their designee; and,
- Must have a commitment to ending youth homelessness, racial equity, inclusion, and issues central to Sasha Bruce Youthwork's mission.

For more than 45 years, Sasha Bruce has implemented innovative interventions aimed at bringing an end to youth homelessness in the DC region. Today, its programs in DC and Prince George's County, MD, reach over 6,500 runaway, homeless, abused, and neglected youth, and their families.
About Thrive House
Sasha Bruce Youthwork partnering with The Community Partnership to establish Thrive House, a joint Transitional Housing – Rapid Rehousing Program for 18–24-year-old young adults experiencing homelessness in the District of Columbia. Thrive House will provide up to 9 months of site based transitional housing support and up to 15 months of scattered-site rental assistance.
Position Summary
Under the Program Director, the case manager will provide care coordination, social service navigation and support to clients in Thrive House, with a focus on clients in the transitional housing portion of the program. They will work with the entire team including the Behavioral Health Clinician, Housing Specialists and Employment Specialist to assess and identify needs and appropriate services to ensure success in the program and beyond.
