    PURPOSE OF POSITION: The incumbent

    provides linkages, advocacy and other supports for program participants

    assigned to their caseload.

    RESPONSIBLE TO: Program Manager

    SUMMARY OF DUTIES: Responsibilities to include but not

    limited to the following:

    Establish a positive rapport with program

    participants while maintaining professional boundaries while demonstrating

    respect, honesty, non-judgment and empathy.·

    Serves as a positive role model for program

    participants by demonstrating good work ethics.

    Develops a Case Plan for each new program

    participant within 24 hours of admission addressing assessed needs, participant

    interests, strengths and resources.

    Establishes and maintains case files according

    to HMIS mandates for each program participant. Each case file must entail a

    case plan with goals updated minimally once per month and case notes updated

    minimally once per month per goal.

    Provides an orientation for new program

    participants detailing program rules, participant rights and responsibilities,

    program grievance policy, case management requirements, behavior requirements

    and acknowledgement statement.

    Meets with program participants assigned to

    caseload on a weekly basis and as needed and agreed upon by program participant.

    Document participant meetings and other

    information as mandated in HMIS to ensure they accurately reflect program

    participant's progress, barriers and resources provided.

    Provides coordination and referral to mainstream

    community supportive services. Supportive services include but are not limited

    to mental healthcare, emergency medical care, Narcotic Anonymous, Alcoholics

    Anonymous, legal assistance, job training, entitlement programs and community

    services for seniors.

    Maintains documentation of program participant escrow

    savings accounts and provides participant relative receipts as stipulated in

    the agency standard operating procedures.

    Completes monthly statistical reports and occupancy reports as stipulated by the program agreement.

    Monitors random urinalysis screenings of program

    participants with follow up action plans as agreed upon in the participant's

    case plan.

    Provides Life Skills Training and education, group and individual counseling and crisis intervention support as needed.·

    Attend all TCP and DHS mandated trainings.·

    Work schedule includes evening and weekend hours if necessary for program service delivery.

    Perform other duties as assigned.

    SUPERVISORY DUTIES: This position does not supervise any other staff.

    WORKING RELATIONSHIPS: Regular internal contact with Agency staff and regular contact with outside community agencies.

    KNOWLEDGE AND SKILLS:

    Education: B.A. /B.S. Degree in Social Work/ Psychology, preferred

    Experience: Minimum of two years' experience in human services

    Knowledge/Skills: Strong communication, organizational and interpersonal skills Commitment and interest in issues affecting unhoused , low income under served

    populations and seniors. Demonstrates intiative and strong leadership skills. Computer literacy, proficient in Windows environment
