- Coordinate multidisciplinary patient centered care team approach for identified patient population.
- Manage course of treatment of patients, ensuring quality patient outcomes and efficient resource utilization.
- Conduct initial and ongoing assessments, manage outcomes, ensure continuity of care through discharge planning, utilization of resources and analysis of variances.
- Function as patient, family, and health care team member contact.
- Identify appropriate patients in designated specialty area requiring case management interventions.
- Perform assessment, data collection, evaluate patient needs.
- Assess clinical, psychosocial status and current treatment plans.
- Develop comprehensive multidisciplinary plan of care.
- Conduct timely discharge planning.
- Monitor and evaluate short-term and long-term patient responses to interventions.
- Assess patient/family knowledge, health status expectations, and locus of control.
- Participate in team meetings, care conferences and continuous quality improvement activities.
- Graduate of accredited nursing program; Bachelor's degree desirable.
- 1 year directly related experience.
- Current NM RN license or reciprocal agreement.
- CPR for Healthcare/BLS certification within 30 days.
- Light physical work required.
