    Description

    Position Summary:

    We are seeking a Case Manager Supervisor to oversee social work staff. This role is responsible for triaging and assigning patients to case managers, and coordinating multidisciplinary care team approaches to ensure quality and cost-effective care. The Case Manager Supervisor serves as a resource to patients, family, healthcare team members, and community resources, and acts as a contact point between them. This is a full-time, day shift position. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.

    • Responsibilities:
    • Provide leadership and identify opportunities for improvement
    • Develop and coordinate comprehensive multidisciplinary care plans
    • Collect, analyze, and share critical patient information with relevant stakeholders to inform care plans
    • Assess patient clinical and psychosocial status and consult on treatment plans
    • Collaborate with other departments and health care agencies to support quality care in area of clinical expertise
    • Ensure timely patient discharge planning in collaboration with physicians, RNs, and healthcare team members
    • Monitor and evaluate short and long-term patient responses to interventions and deviations from care protocols
    • Provide instruction to patients and family on care and related needs
    • Ensure continuity of care among all healthcare settings and promote effective patient and healthcare team communication
    • Perform related duties and responsibilities as required
    • Develop efficient organizational structure. Supervise employees and select, train, evaluate, counsel, discipline, and encourage teamwork and professional growth
    • Develop and administer the division's annual budget and monitor expenditures
    • Enhance professional growth and development by participating in educational programs and attending meetings and workshops

    Qualifications:

    • Master's Degree in a related discipline
    • 4 years directly related experience
    • LPCC or LCSW credentials
    • Certified Case Manager (CCM) or Certified Social Worker in Case Management (C-SWCM) credential
    • CPR certification for Healthcare/BLS within 30 days of hire

    Working conditions:

    • No or minimal physical risk in an office environment
    • May be required to travel to various work sites
    • May perform subordinate tasks in high census/volume periods
    • May be required or is required to perform on-call duties
    • May work rotating shifts, holidays, and weekends
