The Criminal Justice Case Manager provides clinical and administrative support to clients with serious psychiatric and/or substance abuse difficulties within the program of Mental Health Partners.

Do you want to utilize your well-defined experience to provide case management, coordination, therapeutic support, and behavioral support to clients with a high risk of recidivism?

Essential Functions:

Assists clients in skill building and resource development in order to successfully live as independently as possible Assists other teams when necessary with outreach, engagement, and paperwork Provides a variety of case management services, under supervision, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations Supports client recovery by developing and enhancing client access to resources for vocational, financial, medical, housing, utilities, food, transportation or basic living needs as appropriate Works collaboratively with clients and other pertinent staff to provide assessment, develop treatment plans and coordinate appropriate care Assists clients in crisis situations to provide appropriate interventions or referrals Participates in team meetings to staff cases and communicates administrative information Facilitates groups, classes and workshops as needed Provides education on daily living skills and encourages client's use of those skills as needed Provides support in the management of the milieu and the work environment Face to face (in the office or in a community setting) or phone contact with clients in order to facilitate progress toward recovery through resource development Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.

Support transition process of clients involved in the criminal justice system to access and engage with MHP services Works with Boulder County Sheriff's Dept personnel Work in the Boulder County Jail, in the community and at MHP sites Network with inter-agency providers, teams and partners on behalf of client care Network with external partners to coordinate access to MHP services Meet with clients in Boulder County Jail to establish rapport, and inform about MHP services and transition plan Meet with clients at different Mental Health Partners locations/in community to establish rapport, and inform about MHP services and transition plan Develop, or enhance, external partner relationsMental Health Partners offers a diverse, quality work environment, a competitive salary and a comprehensive benefits package.

44 per Hour Skills & Requirements What We Need for this Role:

Bachelors Degree in Psychology, Social Work, Counseling or related field Certified Addition Specialist (CAS) or Certified Addition Technician (CAT) | CAC I or II preferred, or working towards it.

Since then, we have expanded to meet the growing needs of the community in both counties, with locations in Boulder, Broomfield and Longmont.

We are the only agency in Boulder and Broomfield counties that provides comprehensive psychiatric services to county residents regardless of their ability to pay.

People served by Mental Health Partners generally do not have access to services in the private sector because they cannot afford to pay the significant fees, and most do not have insurance.

Mental Health Partners has 24 hour emergency psychiatric services and outpatient offices for infants, children, adolescents, adults, and families. Many programs and services are available including school-based programs, substance abuse treatment, rape crisis counseling, advocacy, and education. Mental Health Partners offers bilingual/bicultural services and an in-house pharmacy.

It is estimated, that of the 360,000 residents of Boulder and Broomfield counties, over 60,000 individuals suffer with a form of mental illness.

This number is only compounded when family, friends, and co-workers are affected by the disabling conditions of a person who is struggling with a mental illness.

Annually, Mental Health Partners serves over 17,000 individuals and our Emergency Psychiatric Services Team responds to 10,000 calls. We serve more women than men and thirty-six percent of our clients are multicultural.
