    Description
    White Sands Treatment Center is a leading provider of drug and alcohol rehabilitative services in Florida. We provide evidence-based treatment in a residential setting for adult men and women.

    We are seeking a strong professional to join our team as a Case Manager for our facility in Plant City, FL.

    Notify nursing of all B2B transfers for nurse-to-nurse report and eyes on
    Responsible for collecting all payments and executing Financial Docusigns
    Responsible for entering pending admissions into KIPU and completing Financial Packets as needed
    Meet with all patients regarding admission or financial related concerns.
    Receive medical records for pending admissions and provide to medical director or admission's office.
    Providing orientation by group schedule, therapist assignment, CM and back up CM to all patients after 24 hours
    Responsible for documenting all patient meetings in Electronic Medical Record and tracking sheets
    Responds to incoming telephone inquiries and initiates follow-up communications.
    Responsible for handling patient concerns and complaints in a timely, professional manner
    DC planning halfway through patient's stay
    Attends in-service training, when necessary.
    Keep clinical records secure at all times and maintain confidentiality pertaining to clients and families
    Will accept and utilize supervision
    Participate in Quality Improvement activities and contribute to the welfare of the organization
    Pursue professional development through participation in education and training programs
    Complies with WHITE SANDS TREATMENT CENTER' Policies and Procedures.

    Have good work habits:
    complete tasks in a timely and efficient manner, adhere to time schedules, maintain good communication, demonstrate willingness to accept responsibility and function well as a team member and contribute positively to the morale of staff
    High School Diploma or GED, bachelor's degree preferred / 2 years of experience working in the substance abuse or mental health field
