Essential Job Functions

  • Provide an atmosphere of warmth, acceptance, and respect for individual(s) and their family, while providing oversight and ensuring the safety of the individual(s) in care.
  • Ensure the wellbeing and development of individual(s) in community settings by assisting individual(s) with their physical, social, emotional and daily life skills. And advocates for the individual(s) human and civil rights.
  • Assist individual(s) in acquiring, maintaining, and improving selfhelp, domestic, socialization, and adaptive skills by connecting individual(s) to local supports and services.
  • Act as a role model to bridge communication gaps and facilitate meaningful relationships/social connections within the individual's community.
  • Share necessary pertinent information affecting the welfare of individual(s) while ensuring the confidentiality and privacy of individual(s).
  • Is responsible for the accurate, consistent and thorough documentation of all identified areas of a person's individual program plan, (i.e. activity logs, expense voucher, etc). And ensures the confidentiality and respect of all files and records for individual.
  • Assist the CPS program in identifying and distributing appropriate community outing options for staff to participate in with their assigned individual(s).

Other Job Functions

  • Attend and participate in pertinent meetings and at least 24 hours of trainings annually that will enhance ability to contribute to individual's care and/or growth.
  • Work cooperatively with Program Coordinator, various other staff, individual(s), and their family.
  • Provide supervision as directed in the individual's ISP. No individual should ever be left unsupervised while in the care of the Community Participation Staff.
  • Complete and submit and/or review weekly schedules of activities with staff and the CPS Coordinator. Notify supervisor of any changes in schedule of planned activity as soon as the change occurs.
  • Provide updated copies of driver's license, vehicle insurance and registration of any vehicle(s) used for transporting individuals. The employee is required to have their own reliable and in good working condition in order to transport individuals.
  • Notify supervisor immediately if there is a lapse in vehicle's insurance or if driver's license or vehicle registration is suspended for any reason.
  • Other duties assigned by the Program Coordinator.

Knowledge/Skills/Abilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be flexible with work schedule to attend planned trips when necessary. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education/Experience:

High school diploma or general education degree (GED) required; some college preferred.


Language Ability:


Must be able to effectively understand, read, write, print legibly, speak and do the following using the English language: read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; write routine reports and correspondence; speak effectively before groups of customers or employees of organization.


Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to perform these operations using units of American money and weight measurement, volume, and distance. Follow a budget, coordinate outings, and reconcile receipts accurately.


Computer Skills:

Computer literacy is not required, but is preferred.

Supervisory Responsibilities:

This job has no direct supervisory responsibilities.

However, this position is expected to serve as a leader and mentor for CPS staff in the program or while participating in Special Trips.


Work Environment:

While performing the duties of Community Participation, the employee will be exposed to outdoor weather conditions. The noise level in the work environment is usually moderate.

In the event that unanticipated aggressive behavior manifests from individuals served and de-escalation techniques fail, physical crisis intervention may be required.


Physical Demands:


The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds with assistance.
