    Job Description

    The Hawai'i Pacific Health Medical Group is comprised of over 700 employed physicians and advanced practice providers.

    Together with our four medical centers (Kapi'olani, Pali Momi, Straub, and Wilcox) and more than 50 convenient clinic locations statewide, our nonprofit health system is one of the state's largest health care providers.

    Our network of physicians and specialists work together to provide a distinctive and effective model of coordinated care for maintaining the health and wellness of our patients.


    Straub's Geriatric Medicine physicians provide ongoing care for older people, including home visits and primary care to nursing home patients.

    The department emphasizes coordinating services and identifying the support necessary to maintain patients in their own homes as much as possible.

    Outpatient consultations for memory loss and other specific geriatric syndromes are also available.

    Straub's Geriatric Health Services professionals are trained to perform a comprehensive assessment of patients, provide case management and outpatient social services, assist with applications for financial assistance and coordinate physician and social worker visits to the home or nursing care facility.

    In this role, you will deliver direct patient care, including evaluation, treatment and technical procedures to a specific group of patients, as well as provide health education, referrals to other disciplines, and consultation with other allied health personnel.

    We are looking for someone dedicated and experienced, with excellent care and communication skills and a commitment to our vision of delivering the highest quality health care to Hawai'i's people.

    Minimum Qualifications:
    Current Hawai'i Registered Nurse license. Current Hawai'i Advanced Practice Registered Nurse (APRN) license. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Two (2) years related experience in nursing.

    Preferred Qualifications:
    One (1) year advanced practical nursing experience. Teaching and/or public speaking experience.
