    At *Greenbriar Community Care Center*, we are searching for a *MDS Clinical Care Coordinator (LPN/RN)* to join us.     Essential Responsibilities:

    • Provide professional nursing care, assessments, and evaluations to residents.
    • Manage nursing activities to ensure the highest level of quality care is maintained.
    • Collaborate with a team to coordinate NH/HH functions.
    • Require at least one (1) year of long-term care experience with prior MDS experience.

    Qualifications:
    • Must have a current, valid Louisiana RN license.
    • Excellent communication and leadership skills.

    Other Qualifications:
    Exclusion from Medicare and Medicaid programs not permitted.
    Must maintain eligibility throughout employment.
    Participation in Tuberculosis screening.
