JOB DESCRIPTION AND POSITION REQUIREMENTS

Summary of the Job:
Responsible for coordination of care along the continuum from admission to post discharge.

Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual's health needs.

Available resources to positive and cost effective outcomes are identified.

The Care Coordinator facilitates progression-of-care; and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective.

Care Coordination improves access to services, reduces duplication, improves outcomes, and reduces resource utilization.

Minimum Qualifications:
Graduate of accredited school of Nursing, BSN degree preferred. Current RN licensure by the Louisiana State Board of Nursing. BLS through the American Heart Association required.

Demonstrates exceptional communication and negotiation skills; demonstrates organizational, prioritization and time management skills; self-motivated requiring minimal supervision and ability to work independently.

Demonstrates leadership skills. Strong interview, assessment and problem solving skills. Knowledge of Case Management Society of America (CMSA) and ACM case management standards of practice. Maintains clinical and professional case management expertise. Computer skills in Word, XCEL, MS Office, Outlook, Electronic Health Record.

Preferred Qualifications:3-5 years of hospital adult acute care nursing experience preferred.

Certification by Commission for Case Manager Certification as a Certified Case Manager (CCM) or Accreditation by the American Case Management Association as an Accredited Case Manager (ACM) is preferred.

Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required. Must be able to work with a moderate level of noise.

Physical Effort required:
Constant (67%-100%) - seeing
Frequently (34%-66%)- lifting, carrying, pushing/pulling, reaching, handling/feeling, talking, hearing
Occasionally (1%-33%)- climbing (stairs, ladders, etc.), stooping, crouching
