ThriveKids Student Wellness is a community-based program at Children's Hospital New Orleans with a mission to improve healthcare and academic outcomes for students by partnering with schools to expand access to healthcare services; services include staffing schools with healthcare professionals and providing/facilitating direct patient care services for schools.
For this position, individual will work as a member of the ThriveKids Care Coordination Team providing care coordination services to youth enrolled in Thrivekids.
Individual will have access to Children's Hospital New Orleans specialty and behavioral health services to supplement and resource student health needs.
Care coordinator will also assist in developing a comprehensive guide to external/community behavioral health resources for families.Individual will be a full-time direct employee of Children's Hospital New Orleans and will have the option of a 10-month or 12-month working schedule (compensation adjusted respectively).
As a support resource to school partners, individual will serve as a communication liaison between medical providers, families, and school personnel.
Individual will facilitate re-entry meetings for Thrivekids enrollees that are re-entering school following an extended crisis, extended hospital admission, or other high acuity absence from school to ensure school partners are aware of all follow up care.
Insight gained in the field will contribute to ThriveKids behavioral health programmatic development.Care coordinator has the opportunity for career development at Children's Hospital New Orleans as well as the option to provide student/patient counseling as needed.
