Responsibilities

The primary responsibility of the LTAC Clinical Transition Coordinator (CTC) is to facilitate a seamless transition for patients discharging from a facility setting to the care of an LHC Group agency|facility for post-acute care needs. The LTAC CTC will respond to referrals and determine the level of care required by the patient upon initial evaluation.

Qualifications

• Formal Education: Associate Degree or certification equivalent
• Experience: 1 year
• License, Registration, and / or Certification Requirement: Yes
• One (1) year acute care hospital experience.
• Must have independent means of transportation, possess a valid driver's license and the ability to drive.
• Automobile LIABILITY insurance coverage must be current.
• Current RN, LPN/LVN or RT license in state of practice or other professional licenses as applicable
• Must have computer skills with knowledge of spreadsheet and word processor software.
• Clerical and business machine skills and knowledge of business accounting.
• Good organizational and personal communication skills.
• Understanding of LTAC Industry preferred.
