Description

Job Summary:

Case management involves working with the individual and others who are identified by the individual such as a family member or guardian in developing an individual support plan and services.

Case management works closely with the individual to assure higher on-going satisfaction with the process and outcomes of the supports, services, and available resources.

Minimum Qualifications:
High School diploma/GED with minimum of three (3) years working with targeted population
OR
Bachelor's degree in Human Services related field and minimum of one (1) year working with target population preferred
OR
Master's Degree in a Human Services related field
