    Description
    Manager Care Management at Providence Seaside Hospital (Full-time and Day Shift)
    $10,000 hiring/sign-on bonus for eligible external hires that meet required qualifications and conditions for payment.

    This position manages the development, implementation and evaluation of programs and activities for the Care Management Department and Emergency Department Social Worker.

    The Care Management program provides oversight for the care management services for patients with complex medical, psychosocial, and financial issues.

    This position is responsible for the operations of the care management activities, including meeting budget, quality and resource targets. This position directs the staff engaged in case management activities which includes regulatory compliance with Federal and State regulations. The manager acts as an expert resource to the care management staff, medical staff, nursing staff and rehabilitation staff.

    Has regular contact with individuals calling in from the Providence community with questions related to levels of care and assistance at home.

    Required Qualifications:
    Master's Degree in Social Work.

    Oregon Registered Nurse License upon hire, Or

    Oregon Clinical Social Worker License upon hire.

    5 years current Mental Health, Medical Social Work or Emergency Department clinical experience.


    Preferred Qualifications:
    Advanced degree in a related field.

    5 years Acute care management and care coordination experience in an acute care setting.

    2 years Management experience.

    2 years experience working in a Home or Community Service setting.
