    Absolute Care offers concierge health services using a risk-bearing, PCP-driven care model. We treat the most clinically complex and vulnerable members of the communities we serve, many of whom suffer from behavioral health, substance use, and SDoH challenges. We use population health management tools to employ a holistic approach to caring for the highest utilizers of healthcare services in our comprehensive care centers and in the community. Absolute Care tends exclusively to the needs of the top four to six percent of the population who persistently represent a disproportionate amount of unnecessary utilization and cost, regardless of whether they are engaged with other PCPs. In our more than 20 years of service, Absolute Care has focused on fulfilling the needs of this population. And we have consistently achieved unprecedented outcomes by addressing medical and psychosocial issues, in addition to the hardships of life that can exacerbate chronic health conditions and complicate access to care.

  • The New Member Enrollment Specialist will be responsible for outreaching to potential new members referred by health plans, educating them on AbsoluteCare, enrolling them as a new member, and transporting members to the AbsoluteCare office.

    Duties and Responsibilities:

    ·Collaborate with the Team Lead to execute creative outreach strategies that will:

  • Be in the community as the "feet on the street" to locate and engage potential new members from a targeted list, who might be hard to reach or locateAttract potential members to enroll at AbsoluteCare as their PCPEffectively explain the benefits of enrolling at AbsoluteCare
  • Outreach to families and/or caregivers
  • Schedule potential members for a tour of the center
  • Transport members to the center in your personal vehicle who:
  • Are scheduled for a tour (possibly the same day they are located)Are scheduled for their New Member Appointment (scheduled from the tour)
  • Report and enter daily outreach activities into CRM and update member's /potential member's addresses and phone numbers as needed.
  • Successfully locate hard to find members utilizing creative strategies
  • Enroll new members who choose AbsoluteCare as their new PCP
  • Establish trust and build meaningful relationships with members/potential members
  • Promote AbsoluteCare medical center as the "Go-To" resource for our members instead of using the emergency room or other fragmented higher cost medical resources
  • Instill and promote AbsoluteCare's 5-star customer service philosophy in the medical center and in the community by promoting the AbsoluteCaree Way
  • Qualifications:

  • High School Diploma or equivalent required
  • Extensive knowledge of Philadelphia (zip codes, neighborhoods, homeless shelters, etc.)
  • 2 years of outreach experience, specifically working in community, health, or social services.
  • Must have a reliable personal vehicle and be willing to use for transport (Only candidates that meet this requirement should apply)
  • Must have a valid driver's license and insurance
  • Ability to creatively locate members
  • Personal and professional passion to help improve healthcare delivery
  • Demonstrated respect for and experience with culturally diverse communities
  • Ability to build and maintain relationships with a diverse population
  • Strategic thinker: ability to analyze, develop and implement non-traditional solutions with results, understanding patient population and financial constraints
  • Ability to multi-task, highly organized, and great communication skills
  • Excels in developing real-world solutions and is quick to resolve and overcome obstacles
  • Creative and enthusiastic
  • Computer proficient in Microsoft Office, using emails, entering documentation into CRM, with the ability to learn new computer software
  • Team and customer service oriented
  • Previous Sales Experience Highly Preferred
  • PHYSICAL DEMANDS

  • Daily travel to local regions, using personal vehicle to transport members and search for members
  • Frequent walking required
