- Collaborating with health care providers, providing health screening in the community, completing outreach activities with community organizations, and providing individual client visits.
- Complete billable and non-billable activities and produce documentation needed for successful reimbursement of community health worker services.
- Encourages and supports community members to make concrete steps toward promoting their health and managing their chronic illnesses (e.g., diabetes, asthma, vascular disease)
- Support young families and new parents in connecting to resources that will assist them in developing a successful health outcome.
- Work in partnership with public health nurse following through with identified needs in the individual care plan of the clients.
- Work with adult and waiver case managers in delivering services needed for successful health outcomes for their clients.
- Other duties to include regular attendance of agency staff meetings and staff unit meetings; completion of assignments in a timely, organized and accurate manner; maintaining a safe environment for self and others and regular attendance is essential to the maintenance of a smooth and consistent workflow of the employee's responsibilities and the functions of the Agency as a whole.

- Fundamental knowledge of community resources.
- Knowledge concerning the operation and use of computer equipment, general productivity software (i.e. spreadsheets, word processing, etc.) and human service customized applications and software (e.g. MAXIS, SSIS, etc.).
- Knowledge of data privacy regulations and data retention schedules.
- Fundamental knowledge of office procedures, functions and recordkeeping and basic case management activities and requirements.
