In this role you will get to make a meaningful and positive impact to the lives of our clients, caregivers, and organization and enable the compassionate and empathetic delivery of essential home care services.
Job Summary:
The Care Manager in Non-Medical Senior Care is responsible for overseeing the well-being of seniors, ensuring the highest quality of care, and managing the retention of clients.
The Care Manager will play a key role in rightsizing services, providing care oversight, and effectively managing caregivers to enhance the overall experience for seniors and their families.
Responsibilities:
Client Retention:
Develop and maintain positive relationships with clients and their families to ensure satisfaction
Conduct regular check-ins and assessments to address client needs, preferences, and concerns
Rightsizing Service:
Collaborate with clients and families to assess and adjust care plans based on changing needs and preferences.
Ensure that services provided align with clients' evolving requirements, promoting a personalized and adaptable care approach.
Care Oversight:
Conduct comprehensive assessments of seniors' physical, emotional, and social needs.
Develop and oversee individualized care plans, ensuring they align with best practices and clients' preferences.
Caregiver Management:
Train and supervise caregivers, ensuring they provide high-quality, compassionate care.
Conduct regular evaluations of caregiver performance and provide constructive feedback.
Address caregiver concerns and provide support as needed.
Advocacy:
Advocate for the rights and preferences of seniors in care decision-making.
Work closely with clients, families, and caregivers to address concerns and ensure open communication.
Documentation:
Maintain accurate and up-to-date records of client care plans, assessments, and caregiver assignments.
Document and communicate any changes in client status or care needs.
Quality Assurance:
Participate in ongoing quality improvement initiatives to enhance the overall quality of care.
Collaboration:
Collaborate with interdisciplinary teams, including healthcare professionals, to optimize client care outcomes.
Participate in regular team meetings to discuss client cases, share insights, and address challenges.
Qualifications:
RN, LVN or equivalent experience
Proven experience as a Care Manager or similar role
Outstanding communication skills, both written and verbal
Ability to travel locally between clients homes
