Description
The Mental Health Association of Monmouth County seeks a full-time case manager for its Community Support Services (CSS) program.


The
Partners in Independence (PII), Partners in Independence II (PII 2) Plus (PII+) and Partners in Independence II Expansion (PII2EX) Case Manager reports to the Community Support Services' Supervisor.


Responsibilities:


  • Overall case management of assigned consumers
  • Contribute to the development, implementation, monitoring, and updating of individualized rehabilitation plans pursuant to NJAC 10:37B
  • Perform therapeutic rehabilitative skill development pursuant to NJAC 10:37B
  • Offer services appropriate to meet the consumers' needs pursuant to NJAC 10:37B
  • Perform crisis intervention pursuant to NJAC 10:37B4.4(a)2
  • Coordinate and manage services
  • Complete all required documentation in consumer files on a timely basis
  • Become familiar and involved with all assigned consumers, including contact information, and aware of their their roles and contributions to the consumer's life
  • Assist consumers with utilizing all available community resources to best meet their needs
  • Maximize consumer independence by assisting residents to improve/develop skills needed to function at their utmost capacity in an individual setting. This may be done by performing activities with consumers to model until the consumer can perform the necessary function independently. This may also be done by referring consumers for such instruction in the community
  • Attend Peer Supervision meetings and agency staff meetings
  • Visit all referred consumers in state psychiatric hospitals and attend all treatment team meetings there, if requested

Skills Required:


  • Assess general clinical status, as well as interpret consumer charts when available
  • Identify clinical crises that may require emergency intervention
  • Engage with consumers and the ability to assist in the development of living skills
  • Network with a variety of both formal and informal community resources and supports
  • Educate consumers regarding mental illness and other functional impairments
  • Discuss substance use/dependence issues, as well as educate consumers on these issues
  • Exercise safe driving habits as identified by NJMVC
  • Awareness of clinical resources available and the means required to obtain them
  • Create a crisis plan with each consumer that utilizes all community 24hour services and Partners in Independence staff
  • Educate consumers on how to follow a crisis plan developed by both them and their peer advocate
  • Excellence verbal and written communication skills

Education:


  • A bachelor's degree with a major in mental health, health or other appropriate human services related discipline, with 2+ years' related work experience
