    Job DescriptionSalary:

    The Mental Health Association of Monmouth County seeks a full-time Clinical Coordinator for its Community Support Services (CSS) program. Reporting to the CSS Director, the Clinical Coordinator is responsible for the following:

    Responsibilities:

    • Complete comprehensive rehabilitation needs assessments, individualized rehabilitation plans, and 90-day reviews for all consumers.
    • Contribute to the development, implementation, monitoring, and updating of individualized rehabilitation plans in accordance with N.J.A.C. 10:37B-2.4.
    • Maintain a caseload and provide case management as needed.
    • Perform therapeutic rehabilitative skill development as outlined in N.J.A.C. 10:37B-4.2.
    • Provide services appropriate to meet consumers' needs as per N.J.A.C. 10:37B -4.6.
    • Conduct crisis intervention as necessary following N.J.A.C. 10:37B-4.4(a)24.
    • Coordinate and manage services.
    • Complete all required documentation in consumer files promptly, adhering to policies and applicable regulations.
    • Familiarize oneself with all assigned consumers, including their contact information, roles, and contributions to their lives.
    • Assist consumers in utilizing all available community resources to best meet their needs.
    • Maximize consumer independence by helping residents improve and develop skills necessary for functioning independently in an individual setting. This may involve modeling activities for consumers until they can perform them independently or referring consumers for instruction in the community.
    • Attend supervision meetings and agency staff meetings.
    • Complete USTF initial completion/termination on AWARDS.
    • Assist the Community Support Services Director with administrative duties as needed.

    Skills:

    • Assess clinical status and interpret consumer charts when available.
    • Identify clinical crises that may require emergency intervention.
    • Engage with consumers effectively and assist in the development of living skills.
    • Network with various formal and informal community resources and supports.
    • Educate consumers about mental illness and other functional impairments.
    • Discuss substance use/dependence issues and educate consumers on these matters.
    • Follow "safe driving" habits as identified by the Motor Vehicle report.
    • Demonstrate awareness of available clinical resources and the procedures necessary to access them.
    • Create a crisis plan with each consumer that utilizes all available 24-hour community services and CSS staff.
    • Educate consumers on following the crisis plan jointly developed by them and their peer advocate.
    • Communicate clearly through writing and verbally.

    Education:

    • Master's degree with a major in a mental health field required.
    • Possession of a valid license, such as Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC).
    • Two years' related work experience.
