- Request, obtain, and send medical records.
- Perform intake and follow-up with clients or agencies to ensure clients are accessing needed services.
- Assist with scheduling medical appoints for clients.
- Answer phones, fax, scan, copy, email, and file records.
- Maintain up to date case files.
- Communicate information to patients and case workers.
- Communicate with medical offices and DCFS case workers.
- Complete required trainings.
- Perform other duties as assigned.
- Ability to work independently with minimal supervision.
- Ability to prioritize and multitask.
- Ability to communicate effectively and professionally.
- Strong attention to detail and organization skills.
- Proficient computer skills: Microsoft Office Word, Excel, and electronic health records.
- Understanding of medical terminology.
- Associate degree or at least two (2) years of experience in healthcare or social service setting
- Experience providing services to families with young children is preferred.

POSITION DESCRIPTION:
Responsible for interim case management for children in the legal care and custody of the Illinois Department of Children and Family Services and who are in the substitute care in the areas served by SIHF Healthcare Lead Agency.

WORK SCHEDULE:
