- Perform and document an initial assessment of the patient and family to identify home care requirements;
- Develop the care plan and make adjustments as needed based on changes in the patient's condition;
- Administer medications and treatments as per physician's instructions;
- Provide guidance, supervision, and evaluation of home health aide services biweekly;
- Determine discharge planning needs during care plan development;
- Adhere to confidentiality protocols outlined by the agency and Federal HIPAA regulations;
- Demonstrate proficiency in OASIS completion;
- Complete documentation using EPIC in compliance with agency timelines.
- Active and valid Massachusetts RN license;
- Current American Health Association BLS certification;
- Associate's Degree or BSN (preferred) from an accredited nursing school;
- Minimum of 1 year of direct care nursing experience, particularly in public health or home care nursing; OASIS and/or EPIC familiarity is a plus.
