    Job Description:

    What You'll Do:

    The registered nurse will plan, organize, and lead home care services with a focus on community health education. They will leverage community resources to deliver services that cater to the needs of patients and families within their homes and communities, following home care regulations and payer guidelines.

    More Specifically:

    • Complete and document an initial assessment of patient and family to determine home care needs;
    • Initiate the care plan and make necessary revisions based on changes in patient status;
    • Administer medications and treatments as per physician's prescriptions;
    • Provide guidance, supervision, and assessment of home health aide care on a bi-weekly basis;
    • Recognize discharge planning requirements during care plan development;
    • Adhere to confidentiality standards as per agency and Federal HIPAA regulations;
    • Show proficiency in OASIS completion;
    • Ensure timely documentation via EPIC following agency policies.

    What You'll Need:

    • An active valid Massachusetts RN license;
    • An active American Health Association BLS certification;
    • Associate's Degree or BSN (preferred) from an accredited nursing school;
    • Minimum 1 year of direct care nursing experience, ideally in public health or home care nursing; OASIS and/or EPIC experience is a plus.
