The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers.

Program Description:
Goddard Riverside's five-story residence on West 140th Street provides affordable, permanent supportive housing for 48 previously homeless adults suffering from mental illness.
West 140th Street Residence is the sister program to Corner House Residence.The two permanent supportive housing residences are in West Harlem and provide supportive housing for formerly homeless adults diagnosed with mental illness and/or substance abuse issues.
The building consists of fully furnished studio apartments, offices for the social service staff and building staff with ample common space, including a recreation room/tenant lounge, kitchen, library and a backyard.
On-site services include but are not limited to medication monitoring, case management, money management, entitlement assistance, medical and psychiatric care, substance abuse counseling, work readiness training, meals program, 24 hour/7 day per week front desk security, and group activities.
Purpose of Position:
West 140th Residence is looking for a dynamic individual to deliver effective case management services using a strengths-based, client-centered approach who will work with the social service team to provide case management services to formerly homeless adults diagnosed with a severe persistent mental illness for tenants to maintain permanent housing.
This person must be innovative, creative, detailed oriented, strengths based, with organizational skills that will promote a proactive, high energy work ethic and positive work environment.
Schedule:
Monday through Friday 9:00 am to 5:00 pm
Salary Range:
$41,668-$52,085Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.
Roles, Responsibilities, and Essential Duties1.
Client Service Provide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.
Coordinate and follow up with medical and psychiatric treatment providers.Assist clients in receiving and maintaining entitlements Assist clients in understanding and complying with building rules and government subsidies to maintain residency Maintain case records with documentation of services provided and outcomes, according to specific program procedures.
Responsible for case notes and service plans using an electronic database Awards.Accompany clients on visits to doctors, entitlement offices or other community services Serve as liaison between other service providers and community resources Encourage participation among resistant clients; conduct meetings to introduce and encourage program participation Collaborate with staff in other GRCC programs to coordinate client service 2.
Participate as a West 140th Team Member Participate in weekly staff meetings, case conferences, training programs and supervision meetings.Collaborate to maintain an active resource directory Participate in West 140th events Assist with the design, planning, and implementation of group activities and events Conduct apartment inspections Other duties as required in accordance with agency polices and funding source requirements, including incident reporting, data collection, and audit preparation Qualifications/Educational Requirements Bachelor's Degree required.
Experience working with individuals diagnosed with severe and persistent mental illness, substance abuse and/or co-occurring disorders preferred.Prior case management experience preferred.

Some walking and travel to escort clients to various appointments throughout the city and occasional visits to hospital emergency rooms.
