- Locations: Manhattan, NY
- Start Date: Immediate
- Schedule: Monday to Friday, 9:00 am to 5:00 pm
- Department: Supportive Housing
- Provide case management to assigned clients, including setting and achieving short and long-term treatment goals
- Coordinate and follow up with medical and psychiatric treatment providers
- Assist clients in receiving and maintaining entitlements
- Help clients comply with building rules and government subsidies
- Maintain accurate case records and documentation
- Accompany clients to appointments
- Bachelor's Degree
- Strong understanding of the Housing First model
- Knowledge of severe and persistent Mental Illness
- Familiarity with harm reduction and trauma-informed care
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Bilingual (English and Spanish)
- Experience working with individuals diagnosed with severe Mental Illness, Substance Abuse, and/or Co-occurring Disorders
- Prior experience in Case Management
