- Follow-up on internal and external problems as related to the referral and admission process to ensure rapid solutions. Maintain knowledge of and effectively promotes programs and internal resources.
- Assist with updating, revising, formulating related policies and procedures with assistance from Facility Administrator/Program Director as appropriate.
- Functions as a resource for and provides team members with in-service/cross-training on acute rehabilitation criteria, diagnostic case mix management, coding, etc., as applicable.
- Schedule and conduct presentations and tours of the the acute rehabilitation unit to referral sources, professionals and family members. Ensure consistent marketing of the unit in the newspaper, television and in the community.
- Work directly with the unit Medical Director on the admission of patients. Completes detailed Pre-Admission Screens, according to facility policies and procedures and payer requirements.
- Collaborate with the interdisciplinary team to ensure excellent coordination of care from admission to discharge.
- Provides patient updates to physicians, payers, case managers, social workers and other relevant persons.
- Reviews patient medical charts and understands pharmaceuticals, test results, therapy evaluations, pre-existing conditions, and has a general medical knowledge of the patient.

The Clinical Liaison is primarily responsible for the referral development activity associated with Mercy Acute Rehabilitation Unit and acts as a liaison between internal and external referral sources, and internal operations.
The Clinical Liaison is responsible for establishing relationships with physicians, payers, and other referral sources. This position will ensure appropriate patient placement through clinical assessments and screening in conjunction with the internal team.
Responsible for identifying and qualifying new business development opportunities and overseeing the capture management and proposal development processes.
Conduct sales calls on existing and potential referral sources to maintain a constant flow of referrals and perform problem solving as necessary.
MINIMUM EDUCATION
Required Minimum Education: 4 year Bachelors Degree
Specialty/Major:
RN/Social Work/SLP/PT/OT
LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated Required: Current BLS CPR card, Must be a graduate of an accredited RN/PT/OT/SLP program with current state licensure
Preferred: 3-5 years experience in an acute rehabilitation setting
MINIMUM QUALIFICATIONS
