In our Center for Internal Medicine with departments for pneumology, we are looking for a qualified case manager (m/f/d).

Your tasks:

Management and coordination of inpatient planning of bed occupancy and overall treatment processes
Admission and discharge planning
Planning of post-hospital care, including in collaboration with the social services department
Management/monitoring of length of stay in cooperation with the medical service
Liaison between nursing staff, doctors, therapists, medical controlling, and social services
