    CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded its services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under-insured or un-insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota

    We are searching for a full-time Mobile Outreach Coordinator to join our Grafton, ND team.

    POSITION SUMMARY & FUNCTIONS:

    The Mobile Outreach Coordinator (MOC) for CHSI works closely with the Regional Operations Manager, CEO, COO, and other staff to lead CHSI's mobile intervention unit. The Mobile Unit extends coverage for all CHSI services including medical, dental, and behavioral health. The unit focuses on providing high-quality mobile outreach services to the most remote and vulnerable communities in North Dakota and Minnesota through robust community engagement and service provision. The MOC will be responsible for all technical and operational aspects of managing the mobile van and will ensure that all mobile unit practices are aligned with the Mobile Unit standards operating procedures (SOP).


    DUTIES/RESPONSIBILITIES:

    • Provide technical leadership on mobile outreach activities and improve staff knowledge and capacity in the implementation of mobile outreach programs.
    • Manage all technical and operational aspects of managing the mobile van. This includes scheduled maintenance, driving, fueling, registration, insurance coverage, emissions compliance, etc.
    • Establish relationships with CHSI communities in Rochester, Moorhead, Wilmar, and Grafton both directly and through close collaboration with the PRCs at CHSI sites. Engage target communities through frequent meetings and joint planning sessions for outreach activities.
    • Conduct a rapid needs assessment and stakeholder mapping of all CHSI communities and make recommendations for community engagement and outreach. This map will include community leaders, religious leaders, other care providers, partners, collaborators, government actors, community-based organizations, and advocate groups
    • Develop a work plan for all activities of the mobile intervention unit and provide regular updates on the work plan to the management team of CHSI.
    • Establish and nurture relationships with potential CHSI partners working towards similar objectives.
    • Lead the planning, implementation, and delivery of outreach activities. This includes planning rosters with CHSI providers, collaboration with PRCs, joint planning with communities, and mobilization of patients for all outreach activities. Potential target community groups include seasonal, migrant, and permanent workers in factories, canneries, and farms.
    • Maintaining a calendar of outreach activities, including community events, workshops, appearances, and other communication opportunities
    • Manage the process of engaging and supervising volunteers to support mobile unit activities as needed
    • Lead the monitoring and evaluation of all mobile unit activities. This includes extensive documentation of all planned outreaches, mobile van-related issues, and success stories. Ensure photos and videos are recorded of all events and ensure all patient care information is captured in the UDS database. Provide quarterly and annual reports as required.
    • Ensure that care provision during mobile outreach events is closely reinforced with health promotion through patient counseling and education. Ensure information and education materials are distributed during outreach activities.
    • Contribute to grant proposal and budget development as needed
    • Support other activities that may be assigned by the supervisor

    PHYSICAL REQUIREMENTS: (Initial by duties you are able to perform)

    • In an eight-hour workday, the employee must be able to: bend and crouch occasionally, sit continuously and stand or walk frequently.
    • The employee must be able to lift up to 50 pounds occasionally.
    • The employee must be able to use hands/arms, and feet/legs for repetitive fine to large motor skill movements.

    DESIRABLE QUALIFICATIONS:

    1. Bachelor's Degree in Project Management, Community Development, Public Health, or related studies; experience in the health sector preferred but not required
    2. At least 3 years of experience in a similar role or carrying out similar duties
    3. Demonstrated track record of successfully working with rural communities in the US and leading community engagement activities
    4. Excellent English language oral and written communication and presentation skills are required. Fluency in Spanish is a plus
    5. Strong interpersonal skills
    6. Must possess the ability to handle multiple priorities in a fast-paced environment
    7. Friendly, enthusiastic, and positive attitude
    8. Proficiency with Microsoft Office Suite
    9. Knowledge and experience coordinating volunteers
