    Description

    Essential Duties and Responsibilities:

    Provide face-to-face substance abuse psychosocial assessmentsDaily review and update of completing assessments, and other duties assigned by program directorFacilitate patient referrals for additional services consistent with assessed needs and physician recommendation.

    Meet patients for follow up reviews or updates as needed or requiredProvide brief interventions, stabilization services and supportive counseling to patientsUnder direction of program director, care coordinator will provide patients medication education print outs that are focused on understanding illness, symptom management and medication complianceComplete and file documentation as required in timely mannerOther Responsibilities:
    Adheres to downtime procedures.
    Adheres to established departmental policies, procedures, and objectives.
    Performs both essential and non-essential job functions in a safe manner as identified by Humboldt Park Health.
    Demonstrates an understanding of and models the mission and core values of Humboldt Park Health.
    Enhances professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.
    Checks hospital email daily.
    Performs other related duties as assigned.
