Overview:

New Mexico Solutions is looking to hire a full-time Care Coordinator for new Health Home Program

Job Summary:

Provides comprehensive care management, planning and coordination of all physical, behavioral, and support services for identified members.
Participates as an active treatment team member serving individuals with behavioral and/or physical health issues.
Coordinates services for eligible clients with diagnoses of severe mental illness or severe emotional disturbances.
Services may include monitoring and coordinating treatment, care management and transitional care, individual/family support, and linking to community services/resources.
Completes comprehensive needs assessments and care plans.
Meets all applicable program regulations outlined by funding sources.

Minimum Qualifications:

Registered Nurse licensed by the NM Board of Nursing with at least two years of relevant behavioral health experience OR;
Bachelor's degree and/or two years of relevant healthcare experience OR
Licensed by Regulation and Licensing Department as behavioral health practitioners
