    Care Team Coordinator

    A healthcare organization is seeking a professional Care Team Coordinator. The coordinator must be skilled in patient assessment, care planning, and resource management. The coordinator will liaison with physicians, nurses and other health care providers to provide quality and cost-effective care to patients. This role requires a thorough understanding of patient care systems, policies, and protocols. The coordinator will be responsible for the timely collection, review, and analysis of patient data, and will assist in the development of comprehensive multidisciplinary care plans. The coordinator will ensure continuity of care through effective discharge planning, utilization of resources, and analysis of variances. Moreover, the coordinator will assist in ongoing evaluation, monitoring, and reporting on patient outcomes, attending meetings and representing the organization in hospital or community committees as required. The successful candidate must have RN certification, one year of relevant professional experience, and a Bachelor's degree in Nursing.

    Responsibilities:

    • Develop and coordinate a comprehensive care plan for the patient population in accordance with individualized patient care requirements.
    • Ensure that care delivery reflects the principles of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable.
    • Identify appropriate patients within the designated specialty area requiring patient case management interventions.
    • Collaborate with physicians, nurses, and other healthcare providers to ensure effective patient outcomes within established time frames and with efficient utilization of resources.
    • Provide timely discharge planning with appropriate resources through addressing patient/family/significant others needs in relation to the medical diagnosis, treatment and resources.
    • Monitor and evaluate short-term and long-term patient responses to interventions, maintain interdependent follow-up as necessary.
    • Promote effective communication with patients, their families and significant others, and the healthcare team, and provide patient and family instruction based on identified learning needs: assess patient/family knowledge, health status expectations, and locus of control.

    Qualifications:

    • Program graduate
    • Bachelor's Degree in Nursing
    • Nationally Accredited Nursing Graduate
    • One year of directly related experience
    • RN certification in the appropriate state or as allowed by reciprocal agreement
    • CPR certification for Healthcare/BLS before hire

    Physical Conditions:
    Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.
