- Facilitate various services for enrolled youth, including transportation when necessary, team coordination, needs and strength assessments, development of individualized care plans, crisis plans, and advocacy for the youth and their families. E
- Coordinate care for youth and families using mental health (CCS) and disability (CLTS) services in collaboration with other CSC service providers to ensure comprehensive interventions to meet their goals. E
- Monitor and negotiate services, modify care plans when necessary, and ensure compliance with legal and organizational requirements. E
- Attend meetings, engage in professional development activities, and maintain communication and collaboration with stakeholders. E
- Bachelor's Degree in Social Work or related field; Certified Social Worker certificate (Preferred) or Master's Degree in a related field (Highly preferred).
- 1 year of experience in mental health services.
- 2 years of experience with youth and young adults with mental health and developmental disabilities (Preferred).
- Bilingual in Spanish and English (Desirable).
- Organized individual with strong people skills.
- Proficiency in Microsoft Office suite.
- Excellent communication and documentation skills.
- Background check clearance.
- Valid Wisconsin Driver's license, auto insurance, and compliance with driving standards.

As a member of a multidisciplinary team, the care coordinator plays a vital role in providing care coordination to youth and young adults with complex needs who have dual diagnoses of mental health and developmental disabilities. The primary aim is to assist them and their families in creating a personalized plan to achieve their goal of happiness, health, and safety.
PRIMARY RESPONSIBILITIES:
